Please initial every place you see ____ , and sign and date at the bottom. You will turn in a signed copy of this form, hopefully before you go on the visit. You will be given an unsigned copy for your records. Suggested dress is slacks or jeans and plain t shirt or dress shirt. Take your cell phone, a picture ID and a little bit of money, a pen and a small  pad.  Suggest you avoid a large purse or backpack.

____ If I visit the two sites on a weekday, I will visit during regular business hours, between 9 am and 6 pm.

____ If I visit the two sites on a weekend, I will visit the two sites on a Saturday between 11 am and 3 pm.

____ I will dress modestly and appropriately so that I do not draw attention to myself.. I will not wear large jewelry, shorts, cutoffs, or tank tops.

____ Upon returning from the site visits, I will as soon as practicable, but certainly that same day, jot down notes about what I saw and heard.
